Preparing to Fill Out Forms

Make sure the person changing their name qualifies.

  • The person changing their name must be live in the county where the petition is filed
  • The person changing their name must be 18 years of age or older.  Or have their parent(s) or legal guardian(s) file the petition for them.
  • The name that the applicant is changing to cannot be a curse word, racial slur, or something considered obscene or offensive
  • Please be aware - Name changes for minors under the age of 18 can be tricky. The state of Colorado requires both parents to be notified of the proposed name change and hopefully both agree to it. A court hearing is generally required in these cases.
  • Please be aware - There are more limited options if the person changing their name has been convicted of a felony. Or committed a crime that would have been a felony before they turned 18. We can help you generate documents if the person changing their name falls in this category. But the court may not approve the name change unless they are changing it back to the previous name or there is a critical reason for the change.

Before filling out our forms you will want to gather the following pieces of information. These forms can take a little time to fill out. We recommend setting aside an hour or two for this process and having all the information by you when starting.
  • You will need the following information for the person changing their name:

  • Current and new name
  • Any past names used
  • Age and birth date
  • Home and mailing address (if different)
  • Colorado county they live in
  • Contact email address and phone number(s)
  • Social Security Number (We will not ask you for this. But you will need to write it on some of the forms when printed)
  • Felony history if they have one. Or information about any crime they committed as a minor if it would have been a felony for an adult.
  • We recommend filling out all the forms you need at one time.
They will not be filed at the same time, but you will have everything you need to do the next step of the process when the previous one is complete.
